The following instruction is applicable for anyone who wants to fill out the IRS Form 941-X. Follow the step by step guide below.
Step 1: Get the softcopy of the form at the official website of the Internal Revenue Service of the Department of Treasury. You can also download the filling instructions to make it easier. Open the downloaded copy on Wondershare PDFelement and start filling it out with the program.
Step 2: On the top left of the form, enter the information required on the boxes. Enter your employer identification number, your real name and your trade name if you have any, and your address. Your address should contain number, street, suite or room number, city state and zip code. If your address is a foreign country, then enter the foreign county name, the foreign province or county and the foreign postal code.
Step 3: On the top right side is a block of entries to be made. Check the type of return you are correcting. It should be Form 941 or form 941-SS, then check the one quarter you are correcting. Check box 1, if first quarter, check box 2 if second quarter, check box 3 if third quarter and check box 4 if last quarter. Then enter the calendar year you are correcting and enter the date you discovered the errors.
Step 4: Go to Part I and select only one process. The process are the adjusted employment tax return which must be checked if you underreported amount or over reported amount and if you are correcting both over and under reporting on the form. The second process is claim. You must check the claim box if you over reported amount only and want to use the claim process to ask for refund.
Step 5: Proceed to Part 2 and complete the certifications. There are certifications statements on line 3 to line 5, read them carefully and check the boxes as required. Note that some statements are dependent on what you checked on Lines 1 and 2, therefore ensure you read and understand the statement properly.
Step 6: Go to Part 3 and enter the corrections for this quarter. But note that if any line does not apply, then leave it blank. You will enter the corresponding amounts in column 1, 2 and 3 as well as the tax correction on column 4 for lines 6 to 19b, see further instructions were applicable. However, for line 20, combine the amounts on line 18 and 19a of column 4 and that is the total.
Step 7: Go to Part 4 and explain your corrections for this quarter. If corrections include both over reporting and under reporting then check box 21. But, if any of the corrections involve reclassified workers then check box 22. IRS demands detailed explanation of how you determined your corrections. Step 8: Proceed to Part 5 and sign the form. Put down your signature on the space provided; write your name, date, title as well as your daytime phone number. If you are using a paid preparer to fill this form, then the paid preparer details should be below the signature section.
