A formal Do Not Resuscitate form or document is more on consisting a statement of the declarant or person about not giving him any methods of reviving his life the moment that his or her heart stops beating. This includes a CPR or cardiopulmonary resuscitation and ACLS or advanced cardiac life support. This is to fully respect the wish of the patient which is through this form. If the declarant or patient may not be able to write and fill the form, it is a physician’s job to do it and any other family member of the patient can interfere it. A certified medical power of attorney must be submitted in this case. This form or document literally limits the life of the declarant.
Different countries have their own format of a Do Not Resuscitate Form. Even though, it must contain correct informations about the declarant and authorizing person. Here is a step by step procedure on how to fill the form out using a sample form:
Step1
Obtain the form in the official sites of the state that the declarant belongs. It may also be handed in by a medical institution.
Step2
Start by filling up the space provided for the identification of the declarant. This must be done by writing down the full name of the declarant and his or her complete residency address.
Step3
Provide the security number after the name and address and supply the accurate date of birth of the declarant. Do not forget to encircle the gender of the declarant below it.
Step4
Read the written statement which states all the possible agreement that the declarant should sign on.

Sign and date the form.
Step6
A notary public should also be present while filling up the form. The notary public must supply his or her name on the space provided and a signature and seal.
Step7
The attending physician must also sign and date the form for swearing a statement written in the form. Include the physician’s address completely written.
Step8
An attending ambulance service representative must also sign and date the form upon notification of the form.
Step9
The last part is for the emergency service section for recording purposes. The statement must be filled with the correct details upon the event.
