Step 1: The first thing is to download the form the official website of the Internal Revenue Service including the filling instructions which can also be downloaded on the website. Then open it on the PDFelement.
Step 2: Fill out Part I which is the identification. On this part, you are to write the name of the person for whom you are acting for in the first line. Note that the name must be the same as shown on the tax return. Also enter the identification number and the Decedent’s social security number. This will be followed by the address of the person for whom you are acting for. The address must include the street, room or suite number, the city or town, state as well as the zip code. However, if the address in question is a foreign address, then see further instructions. When the above is done, write the fiduciary name on the space provided followed by the address of the fiduciary which must include the street, room or suite number, city or town, state and the zip code. You also have the option of entering the telephone number. You can fill it if you want.
Step 3: Go to Section A which is the Authority for the fiduciary relationship. Check the applicable boxes from 1a to 1f. For example, if the fiduciary relationship is based on the court appointment on the testate estate, then check box 1a. If it is based on court appointment for intestate estate, then check box 1b. Continue to check the appropriate boxes as you read through 1a to 1f. However, if you check box f which is “Other”, you will need to write down the description of what it is. For 2a, you have to enter the date of death if 1a and 1b is checked and enter date of appointment, taking office or assignment if you check boxes 1c to 1f.
Step 4: Complete Section B- Nature of Liability and Tax Notices. On No 3, check all that applies for type of taxes. The options are income, gift, estate, generation skipping transfer, employment, excise and other. However, note that if you check the “Other” box, you will provide the description of the type of tax which is not captured on the checkboxes. For No 4, check all that applies for federal tax form number. You have options a to h showing form numbers, check the appropriate box. If h is checked, provide the details. Complete the table if line 6 is checked. Enter the year or periods if the 4a to 4h are checked.
Step 5: Go to Part II Revocation or Termination of Notice. Check the boxes 7a to 7c if you are revoking or terminating all prior notices concerning fiduciary relationship on the file with the Internal Revenue Service. Check 7a if it is court order revoking fiduciary authority. Check 7b if there is a certificate of dissolution or termination of a business entity. If it as a result of other things apart from one listed on 7a and 7b, check 7c. However, provide the description on the space provided. For No 8 and 9,see instructions and complete them accordingly.
Step 6: Fill out Part III- Court and Administrative Proceedings. Write the name of court, the address of court, date proceeding is initiated and the docket number of proceedings with the date, time of other proceedings.
Step 7: Complete the form by signing on Part IV. What is required here is to sign the fiduciary signature, the title if applicable and the date.
