A formal Advance Directive Form is normally consisting of several pages, sometimes combined and sometimes separated, that may include full sworn statements, identification of the subject, signature of authorized personalities, and will, instructions and rules. An authorized personality pertains to an official attorney or lawyer of the subject and some witnesses. Every details and rules as well as instructions that are written in the document must be strictly implemented for it is the decision of the subject for his future when he or she can no longer decide for him/herself. It becomes powerful due to the authorization given by an attorney or lawyer when it comes to jurisdiction.
An Advance Directive Form may vary from each country but it should have the same content needed. Some says that it may not be a form, or it can be just a written statement, but most would like to have it in an actual form by their state. So here is a step by step procedure on how to fill a sample Advance Directive Form:
Step1
Read the instructions and description of each part in the first page. Understand well all the rules that may be implied.
Step2
Start by supplying the form all the necessary identity information. Write the subject’s exact full name, birthdates and address of residency.
Step3
There is a statement just below to state on whether the document would be valid for his entire life or within a span of time. If the document has its expiration include exactly the count of years for validity.
Step4
For appointing a health care representative there is a statement wherein the subject must fill the first line with the name of the trusted appointee. Including his or her address of residency, and contact information. Now there should be an alternate appointee for the position if ever something has happened and the first choice would not be able to do the job, it must be also provided on the first parts.
Step5
The limits and or conditions and instructions must be done next, this is more of a statement if the subject would like to include or specify something.
Step6
Input the initials on the space provided for the following requirements. It is a way of agreeing the statement written in each numbers. Sign and date the form once done.
Step7
The next part of the form is about the subjects Health Care Instructions, there are 7 numbers in which the subject may answer with what he wishes and may also write down some instructions to be followed in the future.
Step8
The last part is about signing and dating the form. It is also required for the subject to prepare some witnesses who will also be signing and dating the form lastly. As well as the appointed representative most importantly.

Choose a trustworthy representative that would eventually be in charge of the subject’s life. It will be his dependant whom empowered by the law.

Get sufficient information about the life sustaining treatments which would be stated in the form in order for the subject to choose what is best for him or her.
