An SF-85 Form is consisting of 8 pages wherein the applicant of the form can see from the first two pages the instructions and purpose of the form. It’ll help the applicant filling the form some guidelines and the right way of filling the form out. A box wherein some codes are written is also provided for some information needed in an abbreviated form. The first page of the form, first section is needed to be filled by the Agency who requested for the background investigation, and next is by the federal appointee. There are lots of question in the form which are needed to be filled out accurately and honestly in order to obtain an all throughout investigation for the sake of security. This form is needed to be signed and authorized by the right person in order to clarify and declare the items and details that are written in the form are legitimate.
There are some parts of the form that is somewhat confusing and needs a lot of concentration. Since the form itself is long and has lots of details that are needed to be supplied, one must know how to fill this out. Here is a step by step procedure on how to do so:
Step1
Notice that Sections A to K is not intended for the federal appointee but for Agency Use Only. Do not attempt to fill this part out.
Step2
Number 1 to 6 is needed to be provided by proper identification of the filler. It helps the appointee to rule out his or her identity specifically. Input full name, date and place of birth to obtain citizenship, and other personal attributes that’ll help identification process uniquely.
Step3
Section 7 requires some information about the appointee’s citizenship. This is needed to be supplied by words in the space provided each statement aforementioned. If there is a case that the appointee has a dual citizenship, the appointee still needs to be specific likewise giving accurate informations.
Step4
Section 8 is more one providing the exact information about where are the places that the appointee has lived starting with the most recent one. This must be specific like stating the year of residency, street addresses, city, zip codes as well as the person who knew that the appointee has lived there.
Step5
Section is stating and providing the informations on where the appointee has studied or the schools that he or she has attended. Just like on Section 9, complete and specific details are needed to be supplied. By the end of this part is a space wherein the appointee must provide his social security number.
Step6
Section 10 requires some information on the appointee’s previous jobs or employment activities. There are some codes to be use so be sure to check this instruction out. Input the needed information generously regarding the needed details.
Step7
Section 11 is more on providing some references for your identity like enlisting the people who knew the appointee well. There must be at least 3 persons enlisted.
Step8
Section 12 is asking for some selective service record on which the appointee has joined and providing the registration number as well as the exemption explanation. A rule is applied here for an appointee who is a male and born after December 31, 1959. And about section 13 is more on providing details if the appointee has served US Military, or the appointee’s military history.
Step9
Section 14 is about truthfully stating if the appointee has used or has been active on any prohibited or illegal drugs.
