This is a kind of US Army form in which states about Notice of Delegation Authority for a receipt of supplies. It is used as a record of the receipt of supplies and refers to as a signature record formally. It is when an accountable or responsible person wanted to divert or transfer the authorization into personnel when a request or sign is needed for supplies. The form itself consists of boxes that are needed to be filled out; it requires some identification of both personnel accountable and the person who was designated. Likewise, the supplying personnel should also fill this form out.
Stating a DA Form 1687 may take effectiveness as soon as this form was signed and approved. Designating someone must go through a legal process so this is a step by step procedure on filling out a DA Form 1687:
Step1
Prepare the form needed and read everything that is written and know what and where to write each detail that is asked for.
Step2
When filling out the form start from the topmost requirement. Noting the form the date of filling out is at the most noticeable part of the form so fill in with the correct date.
Step3
The following space or box is intended for the receiving person’s identification which insisted as name and their location. Start by the last name respectively and so on. The receiving party must also indicate if there are any authority for the receiving and requesting the supply in the box. There is a checkbox for this so be careful on checking or crossing them out. Each name f the authority must be written down; across these boxes there is a corresponding space for the signature of each of the stated person.

Next is the space wherein the responsible or accountable supplying officer’s part indicating what type of transfer is wished to obtain, it is done by checking out the choices from the checkbox. A remark is also needed to explain further information about why the request is happening.

The last part of the form is entitled I ASSUME FULL RESPONSIBILITY most likely a sworn statement that everything in the form is authorized and legalized by the person filling out the form. Unit and identification code as well as the account number of the applicant must fill the boxes asking for it correctly. There is only one account number for each DA Form 1687. It is also required by the form the name of the receiving party, rank and contact information.

Lastly the signature of the receiving party is needed on the last box of the form.

A copy of the DA Form 1687 signed and approved must be kept for further use. Both recipient and supplying party are entitled to have a copy of it for future reference.

Write legibly and accurately. It must be readable since it is going to be kept and used in the future.

Choose the latest form to fill out. It can be looked up on the internet or may be handed by the command officer in charge.

In the official DA Form 1687, there is a dropdown menu for the date or calendar date making the fill out much easier and in the new official DA Form 1687 hand written signatures are no longer needed but a digital type of signature which is made due to the lack of time of some officers.
