The DA Form 2166-8 is a standard United States Army document which is known as the Non Commission Officer Evaluation Report used to record the annual achievement of army personnel in the annual evaluation exercises. Due to the fact that promotion, assignment as well as personnel actions are determined by people who do not know you based on your record, the Non Commission Officer Evaluation Report is probably one of the most single most important documents for a soldier in their entire military career. Step1
The form is a 2 page form document divided into 5 parts. It is expected that you go through the form thoroughly before filling it.
Step2
The first part known as the Part I is the Administrative Data. You will need to fill up the boxes on this part starting from a to O. This part contains mostly of the personal information and it include your name, last, first and middle initials in that order, your social security number, rank, date of rank, your unit, organization, zip code and the major command. Other information required to be filled out in the first part also include the reason for submission and also the period the evaluation is capturing. Note that as a Non Commission officer, you are expected to fill in your email address on the .gov or .mil platform.
Step3
Part II is the place for authentication. The information provided in this column includes the name of the Rater. As usual, the name should start from the last to first to middle initials if any. Also included is the social security number of the Rater, the signature, the date of the exercise, the rank, organization, duty assignment and all other information required of the Rater. Note that there is hierarchy of authentication in the DA Form 2166-8 Non Commission Officer Evaluation report form. For example, besides the Rater, there is also a senior Rater and he or she is expected to fill out his personal details on the form. After all these, there is a reviewer who supersedes the Rater and senior Rater. He or she will either concur with the ratings of the Rater and senior Rater or disagree with them. At the end of this part , the Non commission officer being rated is expected to put his or her signature and date on the authentication part.
Step4
Part III of the DA Form 2166-8is known as the Duty Description part. The information to be filled out in the boxes on this part include the principal duty title, daily duty and scope, areas of special emphasis, appointed duties and counseling dates which include the initial and the latest dates.
Step5
Part IV actually talks about the Army attributes, Values , Skills and Actions. In this part, the check boxes are expected to be ticked yes or no against all the values listed on the form. It is important to ensure that adequate comments are made to support or buttress the value ratings of the Non Commissioned Officer.
Step6
Part V is the final part of the DA Form 2166-8 and it is overall performance and potential of the Non Commission Officer. Here, the Rater and the Senior Rater conclude on the evaluation and issues an overall performance rating.
