Tips for Filling the Template

Filling the DD Form 2875 is actually easy. However, it requires proper and meticulous effort in order to avoid making mistakes. Below are the instructions for filling out the form properly.

1. The form is divided into four major parts. Before filling the major parts, the use is required to tick the type of request, which could be initial, modification or deactivate. The user should also put his or her username, write down the date, fill the system name and the physical location of the system.

2. Part I is expected to completed by the requestor.

3. In block 1 and as a requestor, fill in your full names which include your first, middle initial and last names.

4. In block 2, you are expected to fill in the name of the organization.

5. Blocks 3,4,5,6 and 7 are self explanatory. You are expected to fill in the department, phone, official email address, job title, grade and official mailing address.

6. Block 8 and 9 are check boxes for citizens and designation of person. In the citizen column, you are expected to tick any of the checkboxes that is applicable to you. For designation of person, you are required to tick any of the military, civilian and contractor checkboxes.

7. The training and awareness certification requirement is to verify the completion of the information awareness training. Ticking the checkbox indicates that you have completed the annual information awareness training as seen in block 10.

8. There must be a digital signature in block 11 rounding up part I of the form with the date on block 12.

9. Part II of the DD Form 2875 is the Endorsement of Access by information owner, user supervisor or government sponsor. It should be filled out with care and ensure all provided information is valid.

10. Block 13 is a space for the justification of access. For example, if requested access is for retired casualty pay system, then block 13 is expected to state the following: “Assistant retirees and survivors with reviewing and maintaining system data accuracy for their retired pay account”.

11. It is important to note that Blocks 14, 15 and 16 should have “X” in the “Authorized” box , “unclassified” box and the “ I certify that this user requires access as requested” box respectively.

12. Block 17 to 20b should be filled out by the user’s supervisor. It includes the supervisor’s name, supervisor’s signature, supervisor’s organization, supervisor’s email address, supervisor’s phone number and the date.

13. Digital signature is expected on the supervisor’s signature box and the user should enter his or her name in block 26.

14. Block 21 to 25 is the space for the information owner and appointee which include the signature, phone number, organization and date.

15. Block 27 is a large space for additional information and it is optional. Should incase, there are other information that can support the request; it is best highlighted on this block.

16. Part III of the DD Form 2875 is the part where the security manager validates the background investigation or the clearance information. It is represented from Block 28 to 32. The information required here includes the type of investigation, clearance level and other information. It should be completed by the user’s security manager.

17. Part IV is the final part of the DD Form 2875 and it is normally left blank by the user.

What is DD Form 2875
A DD Form 2875 is a department of defense document known as System Authorization Access Request. The department of defense uses this form to check and validate the level of trustworthiness for any individual who is requesting access to the department of defense information and systems. The user of the form will need to state the type of request to be granted. The request could be initial request, modification or a deactivation request. It is expected that the requester must release their personal information in order to verify their identity. It is important to note that an endorsement for this access must be provided from the information owner. The person is also needs to provide identifying information and to sign the form.
Warning and Tips for DD Form 2875

1. Do not in any way scan the form as it removes the ability to annotate and digitally sign the form especially for people who do not have digital signature capabilities.

2. It is important to note that new users must file DD Form 2875, the DPAS security roles form and the user agreement to receive system access.

3. Note that system name is the actual application name and location is the DPAS server. In the field, enter “DPAS” and “DECC-Ogden” in the system name and location field respectively.
