The DA Form 705 is a document where the result of the Army Physical Fitness Test result is recorded. It is important to note that all active soldiers in the active army, the army reserve and the national guard must take the Army Physical Fitness Test irrespective of their age. Note that the APFT is a program used by the US military to measure the physical fitness and weight control for her members. It is a three event physical performance test taken by soldiers at least twice in a year. The DA Form 705 now serves as a score card for this test where one scorecard will be maintained for each soldier.
Below are the instructions for filling the Da Form 705
Step1
Write the soldiers last name, first name and the middle initial name in NAME block.
Step2
Write the soldiers social security number in the SSN block.
Step3
Write M for male and F for female in the GENDER block.
Step4
Write the soldier’s unit designation block in the UNIT block
Step5
Write the date the APFT is administered in the DATE block.
Step6
Write the soldiers grade in the GRADE block.
Step7
Write the age of the soldier on the date the APFT was administered on the AGE block.
Step8
Write soldiers height in the HEIGHT block. It is important to note that height must be recorded to the nearest inch. For example, if the fraction of height is less than half inch, round it down to the nearest whole number in inches.When it is more than half inch, round it up to the highest whole number in inches.
Step9
Write soldiers weight in the WEIGHT block. Like height, weight will be recorded to the nearest pound. If the fraction of weight is less than half pound, round it down to the nearest pound but if higher than half pound, round it up to the nearest highest pound. Circle GO when the soldier meets the screening table weight or NO-GO when soldier exceeds screening table weight.
Step10
If the soldier exceeds the screening table weight, you are expected to write the soldiers body fat in the BODY FAT block. Circle the GO if the soldier meets the percentage body fat for their gender and age or NO-GO if the soldier has exceeded the percentage body fat for their gender and age. In cases where the soldier does not exceed percentage table weight and does not have excess fat, write N/A which is “not applicable” in the BODY FAT block.
Step11
The event scorer will record the number of repetitions of push-ups correctly performed in the PU Raw score block and write his or her initials in the INITIALS block.
Step12
The event scorer will record the number of repetitions of sit ups correctly performed in the SU RAW SCORE block and write his or her initials in the initial block.
Step13
The event scorer will record the 2 mile run time in the 2MR RAW SCORE block. Note that this time is recorded in minutes and seconds. Then, the event scorer will record the point value for the two mile run by determining it using the standard on the reverse side of the form. Note that the point value will be recorded in the 2MR POINTS block and the scorer is expected to put his or her initials in the INITIALS block.
Step14
The event scorer writes the alternative aerobic administered in the ALTERNATE AEROBIC EVENT block. The recording is done in minutes and seconds when the soldier completes the alternate aerobic event.
Step15
After all the information required is filled, the NCOIC/OIC is expected to check the scores for accuracy and thereafter sign on the signature block.
Step16
If the event scorer, supervisor, NCIOC and OIC intend to record some comments, it should be done in the COMMENTS block.

As an event scorer, ensure that you understand how to determine the standards on the reverse side of the form to ensure accurate reporting.

Filling the form in itself is not difficult. However, effort must be taken to study the form properly before filling it as any mistake could render it useless.

Soldiers are not permitted to administer the APFT themselves in order to record the DA Form 705 just to simply satisfy record test requirements. Doing this will attract consequences.
