A DS-4085 Form is a one page form and also known as an Application for Additional Visa Pages form. It is a very useful type of request for a person who travels a lot. When going in and out of the country, blank pages in the passport will be stamped. Travelling more often means getting a stamp more often as well.
In the first part of the form, the applicant need to check the box that says 48 pages, if not 24 pages will be automatically given, next there are fillable boxes wherein you need to put your personal details such as complete name, birthday, contact information etc. The last part of the form, you need to sign it and put the date of application.
Knowing how to fill out this kind of form will make it easier for the applicant. It can save up time and can avoid mistakes as well.
Step1.Download the Form
There is a downloadable form which anyone can find on the internet. Download the form in an A4 type of clean paper.
Step2.Complete the Form.
Once you got the form, you can now start filling it. Start with the check box above if you want to apply for a 48 page passport. Each box should be filled with only one letter. Continue filling out until on the mid part.
Step3.Sign the form.
On the last part of the form, there is a space where the applicant must sign to indicate authorization of the filled form. The most last part is for the attending officer so keep it blank.
Step4.Submitting the Form
Once you have finished the form, submitting is next. Now in the form you indicate the mailing address where the passport will be sent back to you. When mailing the form be sure to send it along with your passport and the fee for the processing. There are mailing addresses provided in the form so the applicant would know where to send it.
Step5.Receiving the granted request
Once the form and the payment have been received, the applicant must wait for it to be processed. The status of the request can be checked online. Normal processing may take between 4 to 6 weeks.

Avoid mistakes. When filling the form, be sure to use a black pen and write eligibly.

Read first. So this one should always be done. Read the terms and agreement before signing anything.

Complete and Accurate Details. These kinds of form need to be accurate and one must always give the right information. Complete the information needed in it so there would be no further problem. Gathering all the needed items before filling them out should help.

Travel Plans. This one is not really required but may play a big help when an applicant wants the process to be done faster than the usual. This can be a travel itinerary or ticket from an airline or agency.

Expedited Processing. There is an additional fee for a faster processing of the request. In the form, there is a designated address for this. Be sure to check this out.

Payment. When mailing the form, the applicant should not send cash along with it. Checks are acceptable but be sure that the check must have a printed address and not a temporary or starter check.
