DS-64 Form (Statement regarding a Lost or Stolen Passport) is some kind of a report form wherein the person whose passport was stolen or lost is going to fill out. Just like any other cases on getting a passport, he/she will be also given a DS-11 Form which is its conjunction form to obtain a new one. This form consists of tables and sections, including blank space to put in your full name, address and so on for Identification. There are boxes to be checked to indicate what happened, lost or stolen. The details on the reported passport is also needed, some explanations how and why it was lost and the date that the person lost it.
Losing a passport can be a very disturbing thing. One must not panic and act quickly. Unlike applying for a passport for the first time, this can be a little bit faster. There are steps that are required to do so for a convenient processing.
Step1.
A person may go to the nearest police station to ask them to make a report which can serve as supporting paper for the form. There is a check box in the form which asks if a person had reported it to the police.
Step2.
Complete, sign, and mail Form DS-64 to the address on the form so as to inform the U.S. Department of State as soon as possible to cancel the passport officially and avoid further use if the passport was handed into wrong person.
Step3.
Fill out DS-11 form for the processing of the new passport. This is mandatory since every transaction must submit this kind of form which is the form of passport application for replacing the lost or stolen identification card.
Step4.
The applicant must prepare all the government IDs that he has; this may include birth certificate, proof of citizenship and must also bring a recent photo (2x2, colored) and a photocopy of each. Must also bring the travel itinerary or the plane ticket when currently overseas.
Step5.
Go to the nearest passport acceptance facility (Department State) so you can have the report form and submit it personally.
Step6.
Of course one must prepare for the fees that are needed to be paid. Once the form is done and the payment was settled, the person may now wait for the passport to be delivered or to be issued. Remember to include delivery address and sign the form.
When a passport is lost or stolen and will be traveling sooner
Take immediate actions. Report the incident right away, go to the accredited department and apply for the form in personal.
When a passport is lost or stolen and will not be traveling
Just follow the steps since there will be no reason to hasten the process.
If a traveler lost his passport during his out of the country trips, she/he must do immediate actions like filing a police report as soon as possible. Then you need to make an appointment with the nearest US Embassy or Consulate. And bring all the materials for applying a new passport. Remember to do it in person.

When doing a police report, the owner must include a note on how or where to give the passport back if it was lost. This can make the process short can make the trip a little bit hassle free.

Make sure to use a black pen and legibly fill out the form, read each needed details carefully to avoid mistake. Most important be sure to put accurate statements so that it will be processed easily.

Making a duplicate or a photocopy for each document is very useful. Do not forget the supporting papers and most important the passport application form (DS-11 Form).

Find out in advance what are the payment methods available, some of them do not accept credit cards. Being able to find the easiest way of paying is a big convenience.

In some cases if a person is unable to pay for it, he/she will be given a chance to use Financial Assistance wherein they would ask the person to give names of people that they know to help them.
