Open your Excel to create a new project. Select the "Insert" tab, and in the Illustrations group, choose the "SmartArt" tab.

Click on "Hierarchy" from the Choose a SmartArt Graphic gallery. Then choose the organization chart outline and then hit "OK."

Choose a box and write in the text by clicking the [Text] label in the box and then type the information that is needed in the box.

Add more boxes you need to complete the hospital organization chart by clicking on the box next to where you would like to add the box and select from the following:
"Add Shape Before" which adds the box on similar level of hierarchy;
"Add Shape After" which is the position on the hierarchy;
"Add Shape Below" which adds a box a level below;
And "Add Shape Above" which puts a box on a level above.
Step5
Add the assistants, as needed, by clicking box on the hierarchy and choosing "Add Assistant."

Customize the relationships among boxes by representing them with the suitable line. Choose the "Format Shape" located at the shortcut palette. Choose "Line Style," and then choose either a solid or dashed option.

Choose the right layout of the boxes from the chart. Choose "Standard" that will display the boxes as levels of hierarchical. Select "Both" to show reporting connections, with boxes next to the vertical line. Choose "Left Hanging" to show the reporting relationship with every box on the left side of the line that is perfect if you’re showing several divisions on a chart.

Customize the colors of the boxes to further simplify your hierarchy. Choose the SmartArt graphic, and then choose "SmartArt Tools" from Design tab. From the SmartArt Style groups, click the "Change Colors." Then choose the colors to use for the hospital organization chart.
