Filling out the form is quite an easy task and you need the following information to get started.
Step1
You will need the social security numbers for all the people that are applying for Masshealth. In case of a non- United States citizen, you will need other forms of documentation of the people who are applying.

You will need to provide a copy of your federal tax return for the last year. It is important to state here that if you did not file taxes the past year or you income status have changed, you will need to be ready to provide the information regarding your current income. It could be a recent pay stubs or probably an unemployed award letter.

The home or mailing address of everybody in the household who needs the masshealth insurance. It is important to state that theses addresses are important and must be written individually even if the address remains the same.
Step4
You will also have an option of putting the Masshealth ID number, the name of the facility and the reference ID number in cases where it is applicable.

On the Masshealth fax cover sheet, there is a space for the sender’s information. The sender is expected to put their name and their contact phone number. In case where the application is made even as a group of members, the representative of the members is expected to fill in as the sender with his or her name and contact phone number written.

Always remember to have a copy of any document you mail. It is very important as it serves as a point of reference especially in cases where there is a mix up or complication of the process.

It is important to note that when your Masshealth cover sheet is faxed to the recipient, make sure you follow up with phone calls there is no receipt of a written response in a space of two weeks. Ensure you phone is fully charged before calling as it might take more time than expected.

Write your name on the Masshealth ID or the last four digits on of your social security number on the pages of the application.

Note, if your application is denied, you can follow up with a phone call or at most file an appeal should the issue remains unresolved.
