Medicare is an insurance program by the federal government of the United States for people who are 65 years or older, some people who are younger than 65 but with some disabilities and also some people with an End Stage Renal Disease. Medicare in itself is all encompassing as there are different parts of Medicare which cover specific services. The Medicare application form consist of 4 different parts which cover the hospital insurance, the Medical insurance, the Medicare advantage plan and the prescription drug coverage. It is important to seek knowledge of the different parts of Medicare as each part represents a different kind of coverage. Apart from the initial enrollment period, there are two annual enrollment periods where you can change your plan if desired.
Completing the Medicare application form is not difficult; however, effort must be made to fill in the right information required. The following instructions will help to ensure the Medicare application form is filled properly.
Step1
Fill your social security claim number in the boxes provided in the first row. Also fill in your beneficiary identification code.
Step2
Row number confirms if you want to apply for medical insurance under the Medicare. This is just a confirmation and you are expected to check the yes box beside it.
Step3
The next row is the claimant’s name. You are expected to fill in your full name details. Your full name details should include your last name, first name and the middle initial followed in that order. Note that if you do not have a middle initial, leave it blank
Step4
The fifth row talks about social security. Unlike the first row, this is needed when the social security holder’s name is different from yours. Ensure you input the correct number.
Step5
Next is the mailing address. You are expected to provide a clear mailing address for the application. Your mailing address should contain the number and street, the P>O Box or even the route. Note that your address must be clear and can be located.
Step6
Fill in your city where you reside, the state and the zip code. Next is supplying the telephone number. Your telephone number should be 10 digits including the area code.
Step7
Make sure that any telephone number you supply on the form should be in use.

Sign your signature at the appropriate box the way you sign your official documents at the appropriate box and ensure the date is accompanying the signature. The date should be the month, date and year specifically in that format.
Step9
It is important to note that if the application is not signed because you were unable to sign it mark X, in this case you will need a witness who knows you as the applicant. He should be someone you trust that can defend you any time. The witness is there to show that the person represented in the application is real.
Step10
The signature of the witness, the address of the witness and the date the witness signed.

Row 14 is a space for remarks. You are at liberty to air your views and not be witch hunted. Kindly put your review about the system and program. Ensure you understand your initial enrollment period. It is important to note that you must sign up in the first three months of your initial enrollment period.
