Step2Fill out the important details in the form
Part 1 – Your Information.The first part asks you to fill out your personal details which include the name, last 4 digits of social security number, date of birth, email address, your phone number, and the name of your primary physician.
Part 2 – My Sanford Chart Terms and Agreement.In the second part you will read the Terms and Agreement you have to agree on before you get qualified for membership. Read it carefully.
Part 3 – Your Signature.On the third on the last part is where you need to sign, your relationship with the patient, and the date the form was filled up.
